Passport Requirement for Minors is our topic for today. We’re discussing this due to the recent experience we have encountered when we applied for a passport with a minor.

We realized that passport application for minors requires a different requirements compared to adults so we are writing this article. A Passport application form for Minors must be accomplished first.

Right after filling up the application form, the Parent/s or the Guardian of the Minor must complete all the passport requirement for minors which will be submitted.

IMPORTANT! Minors must be accompanied by the Parent/s or Guardian in applying for a passport. IF the minor is not accompanied by the Parent, additional supporting document must be presented. See “Supporting Documents” section for reference.

IMPORTANT! If minor is over 7 years old, the Parent/s must secure or schedule a passport appointment thru DFA Website.

PASSPORT REQUIREMENT FOR MINORS – CORE REQUIREMENT

Below are the essential passport requirement for Minors which needed to be presented in applying for a passport. Absense of one may result to rejection of application so make sure to complete it before going to any DFA Site to apply for a passport.

  • Confirmed Online Appointment (if applicable) – Children 7 years old and below do not need to have an online appointment. Minors ages 7 year old and below are entitled to avail of the courtesy lane accompanied by their their parent/s and minor siblings. If Minor applicant is over 7 years old, a confirmed scheduled passport appointment must be secured.
  • Accomplished Passport Application Form for Minors. You may download it here.
  • Personal Appearance of minor applicant and either parent or authorized adult companion.
  • Philippine Statistics Authority (also known as PSA) Birth Certificate is required. Local Civil Registrar Copy may be required if PSA Birth Certificate is not clear or cannot be read.
  • Marriage Certificate is required if only one parent is accompanying the child. If parents are unmarried, an Affidavit of Support and/or Consent executed by the Mother will be required if she is not accompanying the child. If Affidavit of Support and/or Consent is executed abroad, it must be authenticated by the Philippine Embassy or Consulate

  • School ID (if applicable).

PASSPORT REQUIREMENT FOR MINORS – SUPPORTING DOCUMENTS

Aside from the core Passport requirements for Minors mentioned above, additional supporting documents are needed whenever the Minor applicant falls into the category stated below.

IF in case minor applicant has No PSA Birth Certificate/Report of Birth yet:

  • If Minor is born in the Philippines – A Certified True Copy (CTC) of Local Civil Registrar (LCR) Birth Certificate authenticated by PSA.
  • If born abroad – original copy of Report of Birth or first indorsement from Consular Records Division

IF in case the minor applicant is not accompanied by the parent/s during the application process

  • Minor applicant must submit a Special Power of Attorney (SPA) executed by the parent/s designating the minor’s companion to assist in the application process. The Special Power of Attorney must be authenticated by the Philippine Embassy/Consulate if executed abroad
  • Passport or Valid Government issued ID of authorized adult companion. See Valid IDs accepted in applying a Philippine Passport.
  • Special Power of Attorney (SPA) or Affidavit of Support and Consent must have a copy of parent/s valid ID and/or passport attached

IF in case applicant is not traveling with parent/s

  • Applicant must submit DSWD clearance and Affidavit of Support and Consent executed by the parent/s. The Affidavit of Support and Consent must be Consularized by the Philippine Embassy/Consulate if executed abroad
  • Passport or Valid Government issued ID of authorized adult companion. See Valid IDs accepted in applying a Philippine Passport.

IF in case applicant is an illegitimate child in custody of the Mother

  • Personal Appearance of minor applicant and mother or authorized adult companion
  • PSA Birth Certificate.
  • Passport or Valid Government issued ID of mother or authorized Adult Companion. See Valid IDs accepted in applying a Philippine Passport.
  • If Minor applicant is not accompanied by mother during the application process, applicant must submit a Special Power of Attorney (SPA) executed by the mother designating the minor’s companion to assist in the application process. The Special Power of Attorney must be authenticated by the Philippine Embassy/Consulate if executed abroad
  • If Minor applicant is not traveling with Mother, applicant must submit DSWD clearance and Affidavit of Support and Consent executed by the mother. The Affidavit of Support and Consent must be authenticated by the Philippine Embassy/Consulate if executed abroad.

IF the minor applicant is an Illegitimate child and mother is deceased / absent and father is unknown

IF the minor applicant has undergone the process of domestic adoption

  • Personal Appearance of minor applicant and adoptive parent/s
  • PSA Birth Certificate
  • Valid passport or valid government issued ID* of adoptive parent/s, See Valid IDs accepted in applying a Philippine Passport.
  • Court Decree of Adoption
  • If applicant is not accompanied by adoptive parent/s during the application process, applicant must submit a Special Power of Attorney executed by the adoptive parent/s designating the minor’s companion to assist in the application process. The Special Power of Attorney must be authenticated by the Philippine Embassy/Consulate if executed abroad.
  • If applicant is not traveling with adoptive parent/s, applicant must submit DSWD clearance and Affidavit of Support and Consent executed by the adoptive parent/s. The Affidavit of Support and Consent must be authenticated by the Philippine Embassy/Consulate if executed abroad.

If the minor applicant is undergoing or has undergone the process of foreign adoption / Inter Country Adoption Board (ICAB) adoption process

  • Personal Appearance of minor applicant and ICAB representative
  • PSA Birth Certificate/PSA Certificate of Foundling
  • Endorsement from ICAB
  • Certificate of Child Available for Adoption/Deed of Voluntary Commitment
  • Placement Authority issued by ICAB
  • Certificate for Issuance of Passport issued by ICAB
  • Clearance for Inter-Country Adoption
  • Child Study Report
  • DSWD clearance

If the applicant is a foundling and NOT for adoption
